My neck suffered due to vari-focals having the close up bit at the bottom. The optician's said I could not get the close up bit at the top so I have to bend my neck backwards to see the connections to a ceiling mounted EM. I have tried the cheap close up glasses but they make me feel sick.
You can get "unusual" lenses. A couple of trades who require something different at the top are airline pilots (due to the numerous switch/indicators in the overhead) and snooker players. (I believe some if their eyes are similar, put their bifocals on upside down.)
A good independent optometrist (optician) should be able to help. My cousin, an independent optometrist, got me a suitable prescription for vari-focals where the reading distance was set for the distance to my lap and the instruments in front of me when I was flying as a PPL. I didn't need any over head bits for what I was flying.
