Have you tried a ramp test on the same RCD?
I've had a lot of problems with RCBOs an "no trip" testers - especially lower rated ones (16A or below) - although I suspect the tester is doing a high currenr L-N test as part of the test and that's tripping the magnetic element of the MCB side rather than the RCD side. If all else fails measure Zs upstream of the RCD and add R1+R2.
