Lighting failure

Maybe a rare incident but I have noted that a number of escape stairs are fitted with PIR control, the failure of which can cause at least a section of the stair to remain in darkness. Maintained lighting may be worth considering.
  • At least two different interpretations of the word "maintained".


    As I read it, there was a failure of maintenance: "It was found the company had failed to ensure a suitable system of maintenance was in place and in particular failed to maintain the lighting within the south middle fire escape stairwell – where Frank’s body was found."


    So there may have been a failure of a PIR detector, or the lamp may have blown, amongst other possibilities.
