Is the diagram / picture incorrect? Should it not show the Neutral/Blue in the middle under the meter and the Line/Phase on the outer part? See attachment

Probably not as intended ... see for example Figure 2.31 in the same publication ...

Thank you for pointing this out, I'm sure that Figure in Appendix C can be revised for the next Edition.
