Generic Risk assesment for switching processes that identifys Arc Flash Hazards

Need some assistance please.  I am in the process of creating a Safe operating procedure for electrical working/switching.  It needs to be weighted towards identifying hazards that may occur as a result of arc flash and how to mitigate and what control measures to use.  I have various literature already and a reasonable grip on the subject but are looking to cross check myself and ensure there is nothing I have missed?  Ideally if anyone has a genic risk assessment for Switching (Either LV or HV) that identifies arc flash as an hazard that would be great.

    You might want to consider investing in some software to calculate the Hazard Risk Category and thus determine the required Arc Flash PPE.  ElectricalOM by ModecSoft has an Arc Flash add in.
